Forms in Docket give you a way to standardize the information your team collects, whether that's a safety checklist, a vehicle inspection, or a lead coming in from your website.
Drivers complete checklists and inspections right from their phone, and clients can submit web forms straight into your leads or dispatch queue. This keeps paperwork out of your trucks and information in one place.
Before You Start
Here are a few things to know before you continue:
- Currently, there isn't a way to download a completed form.
- Web Forms are the only form type you can share outside of Docket via text, email, or embedding it on a website.
- If a form includes an address field, Docket will suggest matching addresses to choose from to help your client or driver avoid errors.
Docket Form Types
There are 4 different types of forms in Docket:
- Checklist
- Todo List
- Vehicle Inspection
- Web
A Checklist form is a simple list of items that must be marked 'Yes', 'No', or 'N/A'. Use this form when you need a quick way to confirm whether specific safety procedures or daily driver tasks were completed. For example, a pre-shift Checklist form can be created for your drivers to complete before they get on the road.
A Todo List form is a simple task-based list where items are marked off when they're completed without the Yes/No structure of a checklist form. Use this form when you need a straightforward set of steps that don't need pass/fail tracking. For example, an end-of-day Todo List form can be created for your office staff before they lock up for the day.
A Vehicle Inspection form is a structured checklist built specifically for tracking vehicle conditions. Use this form when you need to track routine or pre-/post-trip vehicle inspections. For example, a Vehicle Inspection form can be created for routine truck inspections between routes.
A Web form is a shareable online form built for lead and client interactions outside of Docket. Use this form when you need to collect information from someone who isn't a client yet or to gather additional information from current clients who visit your business website. For example, a Web form titled "Request a Quote" can be embedded on your website to send leads to your Docket account.
TIP: You don't need to have a Docket-built website in order to use Web forms. If you do have a Docket-built website, a Web form titled "Contact (Web)" will be created and embedded for you by our Customer Marketing team. You can create and embed additional Web forms on your Docket-built website, if needed.
Create a Checklist Form
A Checklist form lets you confirm whether a safety procedure or daily task was completed, with each item marked 'Yes', 'No', or 'N/A'.
To create one, just follow the instructions below:
- Click into "Forms" from the left menu.
- Click the "+ New" button at the top.
- Click on "Create Form".
- Click on the "Checklist" form option.
- Give the Checklist a Name.
- Give the Checklist a Description, if needed.
- Decide whether to toggle on the Require Form Submit setting; it's OFF by default. Turning this on will add a "Submit" button to the end of the form that your Staff/Drivers will have to click to complete and submit the checklist.
- Click "Continue to Items".
- Click "+ Add Item" to add your first thing to the checklist.
- Give it a Name and Description, and then click "Add" to make it show up on the checklist.
- Repeat steps 9-10 for each item you want to add to the checklist.
- Click "Add Form" to create the checklist once you're done.
The new Checklist form will be added to the bottom of your Forms list, but you can easily find it by using the Category filter in the top left or the Search bar in the top right.
Clicking on the checklist will open up the editor so you can make changes as needed.
Create a Todo List Form
A To-Do List form gives you a simple set of tasks that get checked off as they're completed without the pass/fail structure of a Checklist form.
To create one, just follow the instructions below:
- Click into "Forms" from the left menu.
- Click the "+ New" button at the top.
- Click on "Create Form".
- Click on the "Todo List" form option.
- Give the Todo List a Name.
- Give the Todo List a Description, if needed.
- Decide whether to toggle on the Require Form Submit setting; it's OFF by default. Turning this on will add a "Submit" button to the end of the form that your Staff/Drivers will have to click to complete and submit the list.
- Click "Continue to Items".
- Click "+ Add Item" to add your first thing to the list.
- Give it a Name and Description, and then click "Add" to make it show up on the list.
- Repeat steps 9-10 for each item you want to add to the list.
- Click "Add Form" to create the list once you're done.
The new Todo List form will be added to the bottom of your Forms list, but you can easily find it by using the Category filter in the top left or the Search bar in the top right.
Create a Vehicle Inspection Form
A Vehicle Inspection form is a structured checklist built specifically for tracking vehicle conditions, like odometer readings, tire checks, and brake inspections.
To create one, just follow the instructions below:
- Click into "Forms" from the left menu.
- Click the "+ New" button at the top.
- Click on "Create Form".
- Click on the "Vehicle Inspection" form option.
- Give the Vehicle Inspection a Name.
- Give the Vehicle Inspection a Description, if needed.
- Decide whether to toggle on the Require Form Submit setting; it's OFF by default. Turning this on will add a "Submit" button to the end of the form that your Staff/Drivers will have to click to complete and submit the form.
- Click "Continue to Items".
- Click "+ Add Item" to add your first thing to the form.
- Give it a Name and Description, and then click "Add" to make it show up on the form.
- Repeat steps 9-10 for each item you want to add to the form.
- Click "Add Form" to create it once you're done.
The new Vehicle Inspection form will be added to the bottom of your Forms list, but you can easily find it by using the Category filter in the top left or the Search bar in the top right.
Create a Web Form
A Web Form is a shareable online form built for lead and client interactions outside of Docket, like quote requests.
To create one, just follow the instructions below:
- Click into "Forms" from the left menu.
- Click the "+ New" button at the top.
- Click on "Create Form".
- Click on the "Web" form option.
- Give the Web form a Name.
- Give the Web form a Description, if needed.
- Decide if you want to change the default confirmation message displayed to users who submit the Web form.
- If you want users to land be sent to a specific webpage after they submit the Web form, add the webpage's link to the Redirect Link field.
- Click "Customize".
- Decide which of the available Web form widgets you want to add to the form as fields.
- Click "Update Form" to save the Web form. It'll be added to the bottom of your Forms list like the others.
Web Form Fields
There are 17 widgets, or fields, that can be added to a Web form that'll appear for users to fill out. Clicking on one of them will add it to your form and allow you to customize its settings and order as needed.
TIP: Clicking on the "Add Suggested Dumpster Form Elements" button at the top will automatically add the Name, Contact, Address, Service Address, Dumpster Locale, Dumpster Size, Dumpster Rental Length, and Digital Signature fields to your Web form to create a Dumpster Request form.
At the top of the page you'll see an empty text box. You can add text, hyperlinks, images, and videos to this field so that your leads and clients see the information at the top of the Web form.
The Address field collects a full address or lat/long coordinates. Add this if you need someone to enter a billing/mailing address on a Web form.
The Contact field collects an email address and phone number. Add this if you need a way to follow up with someone by phone or email.
The Date field collects a specific day (and time if you add it from the configuration settings). Add this if you need someone to select a date, like a preferred delivery date.
The Dropdown field lets someone pick an option from a list you configure in its settings. Add this if you need to ask a question with a set list of answers, like which service someone needs.
The Dumpster Locale dropdown lets someone select from the locales set up in your Docket account. Add this if you need to know which yard or region a job should be serviced from.
The Dumpster Size dropdown lets someone select from the dumpster sizes set up in your Docket account. Add this if you need to know which size dumpster someone wants.
The Dumpster Rental Length dropdown lets someone select from the rental length options set up in your Docket account. Add this if you need to know how long a client wants to rent a dumpster.
The Image Uploader field lets someone attach an image. Add this if you need a client to upload a photo of the drop-off location or job site.
The Input field collects open-ended text from the person filling out the form. Add this if you need to ask a question that doesn't fit a dropdown, like special instructions for the driver.
The Lead Types dropdown lets someone select how they heard about you from the lead type settings in your Docket account. Add this if you need to track where your leads are coming from.
The Link field displays a clickable link. Add this if you need to point someone to another page, like your terms of service.
The Multi Select dropdown lets someone choose more than one option. Add this if a client might need more than one service at once.
The Name field collects a first and last name, plus a company name if applicable. Add this if you need to know who's submitting the form and which business they represent.
The Service Address field collects the address where the job will take place. Add this if you need to know where to deliver a dumpster or complete a job, separate from a billing address.
The Service Date field collects the day (and time if enabled in the configuration settings) a client wants a service to start. Add this if you need a client to request a specific delivery date/time.
The Service End Date field collects the day (and time if enabled in the configuration settings) a client wants a service to end. Add this if you need a client to specify an end date.
The Signature field collects a digital signature, along with the date, time, and IP address of the signer. Add this if you need a client to sign off on T&Cs before service begins.
Next to each field is a settings icon that will bring up all of that field's configuration options. Use this to customize each field you add to your Web form, like requiring certain fields to be filled out or renaming fields.
View and Share a Web Form
Once your Web form is configured the way you want, clicking on the "Actions" button in the top right corner gives you a few sharing options to send the form to people and/or embedded it onto your business website. You can also turn the URL into a QR Code for easy mobile access.
You can also click "View Form" to see what the form looks like to the people you send it to.
Assign a Form for Completion
After you've set up a Checklist, Todo List, or Vehicle Inspection form, it's ready to be assigned to an Employee, Client, or Contractor to fill out.
Please Note: You cannot assign a Web form to someone for completion.
To assign a form to someone, just follow the instructions below:
- Click the "+ New" button at the top.
- Click on "Schedule Event".
- Click on "Task" from the options that appear.
- Give the task a Name and change the Task Type if necessary.
- Configure the Scheduling Options section with the date, time, and repeating cadence if applicable.
- Select who should be assigned this task (and therefore its associated form) and if you want Docket to send them a notification about it.
TIP: If you're assigning a task/form combo to a client or contractor, we strongly suggest sending them a notification. - Add additional details as needed.
- Select one or more forms to include in this task.
- Add an address if needed.
- Click "Add Task" at the bottom to create it and notify the assignee if you've turned that on.
The assignee will receive an email with the task's details immediately after it's been created, if you've turned on the notification setting.
The task itself will appear on the Dispatch page, if assigned to a Driver.
If it's not assigned to a Driver but still assigned to an Employee in general, the task will appear on the Schedule page.
TIP: If you want to quickly assign a task/form combo to a Driver, you can do that directly from the Dispatch page by clicking the dropdown next to a driver's name at the top of their task column and clicking "Create Task".
Complete an Assigned Form
Currently, Checklist, Todo List, and Vehicle Inspection forms can only be completed in Docket's mobile app.
Please Note: If you've assigned a form to be completed by a client or contractor, your Office Staff will need to assist them in updating/completing the form.
Your drivers will see Checklist, Todo List, and Vehicle Inspection forms appear in the mobile app just like a regular dumpster task.
To complete them, they'll follow the instructions below:
- Tap "Start Task".
- Tap the first item in the form.
- Enter the required information and/or mark the item as complete.
- Tap "Submit" at the bottom, if it appears (this depends on your configuration settings).
- Repeat steps 2-4 until all the items have been Completed/Submitted.
- Tap "I am done." to complete the task.
Once a driver completes the task and submits the form, the completed form can be viewed by clicking on it from inside the task in Docket.
