Guide to Docket Forms

Docket forms help streamline operations, particularly for scheduling and tracking tasks for drivers. Below is a breakdown of form types, how to create and schedule forms, and how drivers can complete them.


Docket Form Types

Form Type Description
Checklist Items must be marked YES, NO, or N/A. Used for safety procedures or daily driver tasks.
To-Do List A simple task-based list where items are checked off when completed.
Vehicle Inspection A structured checklist for tracking vehicle conditions, including odometer readings, tire checks, and brake inspections.
Web Form A sharable online form for customer interactions, such as booking requests or contact collection.


Web Forms

  • The only form type that can be shared via a link.
  • Can be texted, emailed, or embedded on a website.
  • Used for quotes, contact collection, or customer interactions.
  • Access sharing options via the blue action button next to the web form.

Creating a Form in Docket

  1. Navigate to the Forms page.
  2. Click +New in the top-left corner.
  3. Select Create Form.
  4. Choose the form type (Checklist, To-Do, Inspection, Web Form).
  5. Fill out the basic information, then customize the form as needed.
  6. After creating the form, you can attach it to tasks.

How to Schedule a Form Task for Drivers

Option 1: Using the +New Button

  1. Click +New.
  2. Select Schedule Event > Task.
  3. Enter the Task Name and Date.
    • You can make it a recurring task if needed.
  4. Select the Duration.
  5. Under Attendees, assign the driver.
    • The system automatically assigns the creator of the task, but you can remove yourself if needed.
  6. Under Additional Options, you can add:
    • Description, Attachments, and Location.
  7. In the Forms section, select the correct form (Checklist, To-Do, or Inspection).
  8. Click Add Task.
    • The task will now appear under the driver’s name in Dispatch for the selected date.

Option 2: Scheduling from Dispatch

  1. Open Dispatch.
  2. Click the dropdown next to the driver’s name.
  3. Select Create Task.
  4. Follow steps 3 through 8 from Option 1.

Note:

  • Once a driver completes a task and submits the form, the completed form can be viewed by clicking on the task in Docket.


Downloading Forms

Docket does not currently offer a direct download option, but you can:

  • Take a screenshot, then download/print the form.

Completing Forms in the Driver App

  1. Start the task.
  2. Click More.
  3. Select the form to open and complete it.
  4. Click Submit after filling out the form.
  5. Click Back in the top-left corner.
  6. Click I’m Done, then Done again to complete the task.
